In view of the two failed biddings held last April 18, 2018 and July 4, 2018, the Iloilo Provincial Government (IPG), through its Bids and Awards Committee (BAC), invites PhilGEPS-registered suppliers under Platinum category to participate in the negotiation for the project Proposed Completion of OR/DR Complex at Barotac Viejo District Hospital, Barotac Viejo, Iloilo in accordance with Section 53.1 of the revised Implementing Rules and Regulations of R.A. No. 9184, otherwise known as the Government Procurement Reform Act."

TERMS AND CONDITIONS
1. Contract period shall be One hundred Eighty (180) calendar days.
2. Winning bidder shall start the work five (5) days upon the receipt of Notice to Proceed (NTP).
3. Winning bidder shall inform the office of the Provincial Engineer in writing five (5) days prior to start of work.
4. Contractor shall provide safety precaution during work and shall be liable for the damages to person/property arising from the execution of work.
5. Liquidated damages shall be charged to the Contractor for every day of delay as provided in RA 9184.
6. Work shall conform to the approved plans and specifications.
7. All materials shall be delivered on the job site and inspected by the Project-In-Charge prior to start of work.
8. Contractor must observe cleanliness in the job site upon the acceptance, during and after completion of work.

Terms and Conditions:
1. All entries must be filled-up, written, or printed legibly. Failure to use this form will result to disqualification of your bid.
2. Delivery period: within ONE HUNDRED EIGHTY (180) calendar days upon receipt of the Notice to Proceed.
3. Bids and bid securities shall be valid for one hundred twenty (120) calendar days from the date of the opening of bids.
4. Bids exceeding from the Approved Budget for the Contract (ABC) shall be disqualified.
5. The participating bidder/supplier or an authorized representative must indicate an active email address or working fax number. Absence of both shall be a ground for disqualification. Notice to the bidder through either of these means (email/working fax number) is considered sufficient notice.
6. The supplier must indicate its complete business/mailing address. Otherwise, the lack thereof is a ground for disqualification.
7. The supplier/awardee(s) must conform to the Notice of Award within three (3) calendar days from notice of such award and enter into contract with the Procuring Entity within ten (10) calendar days from receipt by the winning bidder of the Notice of Award. Otherwise, the award shall be forfeited or cancelled.
8. The AWARDEE shall be responsible for the source(s) of his supplies, materials, equipment and shall make deliveries in accordance with the schedule, quality and specifications of award and purchase order. Failure of the AWARDEE to comply with the same shall be ground for the cancellation of the award and purchase order issued to the AWARDEE.
9. All deliveries by suppliers shall be subject to inspection and acceptance by the representative of the General Services Office and End User.
10. Rejected deliveries shall be construed as non-delivery of the product(s)/item(s) so ordered and shall be subject to liquidated damages, subject to the terms and conditions prescribed under paragraph 11 hereof.
11. Supplier shall guarantee his deliveries to be free from defects. Any defective item(s)/product(s) therefore that may be discovered by the End-User within the warranty period after acceptance of the same shall be replaced by the supplier within seven (7) calendar days upon receipt of a written notice to that effect.
12. Performance Security or Warranty Security, as the case may be, shall be required from the winning bidder in accordance with the provisions of R.A. No. 9184 and its revised Implementing Rules and Regulations.
13. When an occurrence of a tie among bidders takes place, i.e., two or more of the bidders are determined and declared as the Lowest Calculated and Responsive Bidder (LCRB), the winning bidder shall be determined by draw lots upon notice to the bidders concerned and/or their authorized representatives in accordance with the relevant circulars issued by the Government Procurement Policy Board (GPPB).
14. All offers/quotations must be properly sealed and marked in envelopes. Non-compliance is a ground for disqualification.
15. The printed name and signature of the bidder/supplier or of its authorized representative must be legible and verifiable and must indicate his/her position. Non-compliance is a ground for disqualification.
16. Bidders shall prepare an ORIGINAL of the Documentary Requirements for Negotiated Procurement. GENERAL REQUIREMENTS
All works to be done shall be with first class workmanship and shall conform to plans and Specifications. All materials to be use shall be of good quality and properly inspected by the authorized representative or the Engineer In-Charge. The Provincial Engineer is reserved the right to rescind, terminate, suspend the contract if he deems necessary and to the best interest of the Government. The contractor shall be liable for all damages that will occur during the construction of the project.

Outline Specifications
I. REMOVAL/INSTALLATION OF ROOFING
All materials shall be of the kind size specified on the plans. All works shall be performed and computed in accordance with the generally accepted and modern practice of roofing. Use materials as specified in the plan. All welding shall be done by approved, competent, experienced and fully qualified welders. Surfaces to be welded shall be smooth, uniform and free from fins, tears and other defects, which would adversely affect the quality of the weld. The contractor shall remove and replace or correct as instructed and welds found to be defective or deficient. He shall also replace all methods found to produce inferior results with methods that will produce satisfactory work. For the installation of downspout use 3- dia. PVC Pipe series 1000 with G.I. strap spaced at 1.0m on center & for the Insulation use double sided with 10mm thick. Use pre-painted long span for roofing. After removal of existing roofing all existing trusses shall be re-painted by red oxide before install new ones. See plans and details.
II. CEILING WORKS
The works include removal of existing ceiling as per indicated in the plan. All existing structures/utilities not to be replaced or repaired shall be protected from any damage by any work being done. Materials to be used for ceiling board shall be 3.5mm thick Hardilite attached to 19mm x 50mm, 0.40mm metal furring channel & 25mmx25mm, 0.40mm Wall angle. Metal furring shall be attached with W-clip to 12mmx38mm, 0.80mm Carrying channel spaced @ 1.20m. All spaced @ 0.40m on wall side the rest @ 0.60m center both ways. Hanger shall be #12 tie wire spaced @ 1.20m on center both ways. Electrical System such as lightning fixtures and wiring shall be restored upon completion of the ceiling works. See plan for dimension and details.
III. TILE WORKS
The works include chipping of tiles as per indicated in the plan. All existing structures/utilities not to be replaced or repaired shall be protected from any damage by any work being done. Tiles shall be soaked in clean water prior to installation for a minimum of one hour. Mortar mix for scratch coat and setting bed shall consist of one part cement and 3 parts sand by volume or use Tile adhesive (Heavy Duty) for mortar. Determine and mark layout of tiles as to joint location, position of trims and fixtures so as to minimize cutting less than one half size of the tile. Thoroughly dampen surface to be tiled but not to saturate the surface. Apply and spread mortar mix for setting bed and tamp to assure good bond over the entire area to be laid with tile. Allow setting bed to set sufficiently, then spread a bond coat over the surface and lay the tile. Grouting mortar shall be applied over the tile by float or squeegee stroked diagonally across the joints. Remove excess mortar with wet sponge stroked diagonally or circular motion after 12-15 minutes. Follow with a barely damp or dry sponge to remove remaining haze while smoothing all grouted joints. Use granite tiles for the installation of 0.10m x 0.60m baseboard molding. Use In-Laid Vinyl Sheet for wall code: 2063 Emerald at 1.0m height from finish floor line with in-laid vinyl sheet mural plus 3011 up to ceiling line with in-laid vinyl for flooring, Code: 2054 Cloud. The color of the tiles shall be referred to the end-user or the Engineer in charge. See plans for dimension and details.
IV. CARPENTRY WORKS
1. Execute rough carpentry in best, substantial, workmen like manner, erect framing true to
Line, levels and dimensions, squared, aligned, plumbed, well spliced, nailed and adequately braced, properly fitted using mortise and lenon joists.
Finish carpentry shall be absolutely kiln dried lumber and water proofed marine plywood. Finishes lumber shall be S2S or S4S.
Joinery details- various types of joints are used for millwork construction to increase the strength of the joint and improve the appearance by eliminating mechanical fasteners such as screws. With the availability of high strength adhesives, screws and other mechanical fasteners are seldom needed for the majority of the work produced in the shop. Field attachment, however, often requires the use of blind nailing or other concealed fastening to maintain the quality look of the work.
Materials to be used for cabinets shall be, A3/4-thick Marine Plywood framed w/ 2-x2- (S4S) & 2-X3- (S4S). For Nurse Desk use A3/4- granite slab for counter top & A3/4- granite splash board with A3/4- Marine Plywood protruding. Provide necessary fittings such as cabinet hinges, catches, drawer guide, and handle (good quality). Apply wood glue to ensure fixedness. See plan for dimensions and details.
For additional drywall partition use 6.0mm thick x 4-x 8- Cement Board on 0.5mm x 35mm x 76mm Metal Tracks and 0.5mm x 32mm x 76mm Metal Studs space at 0.40m on center.
Use fabricated steel louver door with 4-sets stainless heavy duty hinges and steel door jamb to be installed at EE Room.
V. PAINTING WORKS
Paints to be used shall be Boysen, Davies or any equivalent in brand
All painting materials shall meet the requirements of standard specifications and subject for the approval by the supervising engineer. All paints shall be delivered at the jobsite in the original containers with label intact and seals unbroken. Surface to be painted shall be clean smooth and rid of dust, dirt, rust, chalk, grease, wax polishes, alkali and other foreign matter. Old paint shall be remove stripped using scraping, wire brushing and sanding, repair all voids, nicks, cracks with suitable patching materials.

All works shall be done in workmanlike manner by skilled painter. No painting shall be thoroughly stirred so as to keep the pigments spread evenly when paint is being applied to the surface. All coats shall be thoroughly dry first before application of succeeding coat.
The Contractor shall include in his work all clean-up of paint on the floor.
VI. ROOM SIGNAGES
All materials shall be of the kind size specified on the plans. Vinyl Sticker background color green on 3mm thick acrylic sheet with vinyl sticker letters cut-out color white avant garde-book bold font. Use #10 decorative hanging chain chrome pin with # 1- coated cup hook (white) attach at ceiling for the installation of directional signage.
VII. ELECTRICAL WORKS
SCOPE OF WORK
The works include furnishing of labor, materials, tools, equipments and other incidentals necessary to complete the project such as: electrical works.
a. The work under this section consist of furnishing of labor, materials, equipment, Tools and all incidentals to complete and ready for use as indicated in the electrical plans for the project, in accordance with the drawing, specifications and contract.
1. Furnishing and installing all necessary sub-feeder and branch circuit wires necessary with corresponding sizes as indicated in electrical plans.
2. Furnishing and installing all necessary electrical wiring devices such as utilization outlets, wall switches, and receptacle all complete with their proper cover plates.
3. Furnishing and installing all necessary pull boxes, junction boxes, utility boxes and other type of box as site condition requires and /or as indicated in the plan.
b. All work hereunder, shall comply with the latest edition of the Philippine Electrical Code Part I, the rules and regulation of the Local Electric Ordinance of the locality, the rules and regulation of local government authorities and with the Republic Act 7920 as applied or enforced in the projects locality.
c. All installation work shall be done under the direct supervision of a duly licensed Registered Electrical Engineer as provided or employed by the contractor. Electricians undertaking the actual installation shall be as trained and certified by TESDA.
2.0 PRODUCTS
2.1 CONDUITS, FITTINGS AND BOXES
All wirings running through concrete ceiling or concrete partitions shall be in uPVC thickwall conduit, concealed and properly embedded. Conduit shall not be less than 20mm nominal diameter or as indicated in the plan. All conduits fittings shall be of the same brand with the conduit. Proper connection between conduits shall be observed by using PVC solvent cement Utility and junction boxes shall be PVC, heavy duty and deep type with brand the same with that of the conduits. Pull boxes and wire gutters as site condition would require its installation shall be gauge #18 and painted with gray epoxy primer finished in enamel gray paint. Sizes shall be subject for approval by the Electrical Engineer. All junction boxes, square boxes and pull boxes shall be provided with corresponding and appropriate cover.
2.2 WIRES AND CABLES
Branch circuit and main feeder lines shall be laid out in accordance with the diagrams as shown in the drawing. Unless otherwise specified in the specifications or shown in the drawings, type THHN wires shall be used to all branch circuit conductors. The wires as shown in the electrical plan shall be the minimum sizes to be used. Wires shall be as recommended by the Electrical Engineer. For service conductor tap shall be heavy duty, all brass and copper split bolt type solderless connectors and are tightened to the maximum mechanical strength and insulate with rubber friction tapes to ensure an insulation of the conductors they join. Branch circuit splicing and tapping for lighting and power junction and utility boxes shall be done using rubber tape and must be overlay by electrical plastic tape to limit moisture penetration. No splicing shall be permitted inside the conduits. Unless otherwise site condition requires such splicing, proper boxes shall be provided.
2.3 LIGHTING FIXTURE
All Lighting fixtures to be use shall be limited to Compact Fluorescent Lamp type. Outdoor globe luminaire with 18W 3U Compact Fluorescent Lamp shall have holder with full acrylic cover, outside aluminum guard with top cover, properly mount in the surface of the column top. Canopy lights shall be Compact Fluorescent lamp in painted cylindrical steel cover.
3.0 TEST AND ADJUSTMENTS
After the installation have been completed and when the system is ready for operation, insulation resistance testing and continuity test shall be conducted by a competent and experienced engineer to be furnished by the contactor. These tests shall demonstrate the insulation level of the wirings detecting any presence of grounded installations and short circuits. The contractor shall also furnish all the instruments such as multi-tester and insulation resistance tester to enable him to carry out a comprehensive test of the equipment and system.
VIII. AIRCONDITIONING SYSTEM
GENERAL
1. All mechanical work shall conform according to PSME Code of the Philippines.
2. All window type air-conditioning should be installed with 0.5 inclined back.
3. All Split type Air conditioning piping layout must be insulated with charole, rubber insulation and ACU brackets/pedestal.
4. All aircon drain line must be properly installed.
5. R-410A or R22 refrigerant should be used in all Airconditioning units
6. Provision of brackets for the indoor and 900mm ground level for the outdoor unit.
7. Outdoor Breakers should be provided for each Airconditioning units.
8. All aircon installer should be authorized installer of manufacturer.
9. All mechanical installation should be supervised by RME / PME according to
RA 8495 known as ME Law.
1.1 SCOPE
The contractor for the installation of the air conditioning system shall be a qualified contractor and installer, regularly engaged in the installation and must have at least one year experience on the trade. The work shall include the furnishing of all labor, materials, equipment and services necessary the complete installation, testing and commissioning of the system as per plan. In case of conflict between plans and this specification, the Architect/Consultant shall be notified. All permits and fees, private or government shall be secured by owner prior to the start of work. Furnish and install split and window type air-conditioning units as per plan and specification. System shall include piping installation, brackets and supports, circuit breaker wires, refrigerant piping and other to complete installation. Do all necessary civil works in the installation of outdoor and indoor unit window type Refrigerant piping shall be hard/soft drawn Type L, Refrigerant piping shall be insulated with 20 mm thickness cellular foam, flexible, cellular elastromeric molded in the suction lines and covered with canvass clothes or leather as vapor burners.
Drain pipe shall be UPVC of 15 mm schedule 50 and shall be insulated at least 2 meters from the unit with 10 mm rubber cellular elastromeric insulator. All piping shall be bracketed, supported, and hang at least every 3m on BI pipe and every 2m on copper piping. Refrigerant piping shall be pressured tested at least 1.5 times the operation pressure or at least 1.72 mpa and maintained for at least 1.72. Mpa and maintained for at least 8 hours. Refrigerant pipings after pressure testing shall be evacuated using triple evacuation prior to the charging.
SUBMITTALS
Product data/Manuals & Brochure: Within 35 calendar days after the Contractor has received the Owner-s Notice to Proceed, submit : Manufacturer-s specifications and other data needed to prove compliance with the Materials list of items proposed to be provided under this Section Specified requirements. Shop Drawings and other data as required indicating method of installing and attaching, except where such details are fully shown on the Drawings.
QUALITY ASSURANCE

TEST & COMMISSIONING
Submit leak test results (with picture) prior to startup, start-up report should be submitted during and after the test and commissioning period for acceptance.
IX. INSTALLATION OF FIRE PROTECTION SYSTEM
SCOPE OF WORK
The works include furnishing of labor, materials, tools, equipments and other incidentals necessary to complete the project such as: Sprinkler System, Fire Department Inlet/Manifold Connection, Supply of Fire Hose Cabinet with glass & complete accessories (FHC), Miscellaneous & Consumables, Pumps & Accessories, Cistern tank & electrical works, Removal & Installation of Ceiling and Concrete Chipping/Finishing & Painting, electrical works (Fire Alarm Control System), and Testing & Commissioning & Training of Personnel per plan & specifications.
Contractor shall provide safety precaution during work and shall be liable for the damages to person/property arising from the persecution of work.
All materials shall be delivered on the jobsite and inspected by the Project in charge & MQC prior to the installation.
Contractor shall coordinate with Hospital Maintenance personnel and conduct Tool Box Meeting prior to execution of work.
GENERAL
1. All mechanical work shall conform according to PSME Code of the Philippines.
2. All pipe shall conform to ASTM standard APO brand or equivalent.
3. All installer for working should be certified welder by TESDA.
4. Fire Pump / Jockey Pump and all ACC for sprinkler system should be UL / FM Standard.
5. Pressure test at 200 PSI at 24 hours testing to maintain at 120 PSI working pressure.
6. All mechanical installation should be supervised by RME / PME according to
RA 8495 known as Mechanical Engineering Law.
7. Test and Commissioning should be done after the whole system is fully operation (online).
1.1 SCOPE
a. The contractor for the installation of the fire protection system shall be a qualified contractor and installer, regularly engaged in the installation and must have at least one year experience on the trade. The work shall include the furnishing of all labor, materials, equipment and services necessary the complete installation, testing and commissioning of the system as per plan. In case of conflict between plans and this specification, the Architect/Consultant shall be notified. All permits and fees, private or government shall be secured by owner prior to the start of work. Furnish and install fire protection system as per plan. System shall include piping installation, brackets and supports, circuit breaker wires, refrigerant piping and other to complete installation. Do all necessary civil works in the installation of pipings for sprinkler system. Fire protection piping shall be BI pipe schedule 40 and welded connection for 50 mm and above. For 50 mm below maybe BI and threaded connection.
All piping shall be bracketed, supported, and hung at least every 3m on BI pipe Fire protection piping shall be hydrostatically tested to at least 0.6 Mpa Fire protection piping shall be painted with two coats of epoxy primer and finished with two coats of red enamel paints or as per required by code.
Hangers and supports and brackets shall be pre-painted with two coats of epoxy primer finished with two coats of aluminum or silver paint. BI pipe installed shall be painted with two coats and epoxy primer and finished with two coat of red enamel paint.
SUBMITTALS
Product data: Within 15 calendar days after the Contractor has received the Owner-s Notice to Proceed, submit :Manufacturer-s specifications and other data needed to prove compliance with the Materials list of items proposed to be provided under this Section Specified requirements. Shop Drawings and other data as required indicating method of installing and attaching, except where such details are fully shown on the Drawings.
QUALITY ASSURANCE

General
The Contractor shall perform the work described, in the welding installation of all B.I. pipes should be done by a certified TESDA Class I. Pendant or downright type sprinkler be used in the area of open space and sidewall in case maybe. Sway Bracing for sprinkler will be installed in no column available in the area and Pipe bracket will also be installed in the area as to plans and specification. Pipe hanger properly installed to handle the wet type system according to table of dimension diameter. Sprinkler distance at least 290mm equidistance. Siamese twin hose header should be 900mm from the floor elevation with cap and chain strap. Alarm check valve assembly (for wet type) should be installed and supervisory for ground and second floor control valve assembly. Fire Hose Cabinet (FHC) is of surface mounted with 900mm from floor elevation with 750mm with, with 30m double jacketed 1 A1/2-A˜, with 4.5kg portable fire extinguisher and a fog type nozzle and Fire Ax.
All pipes will be of ERW ASTM standard schedule 40, sprinkler head will be UL/FM standard, hydrotesting should be at 7 bars at 24hrs. Testing and Commissioning shall be witnessed by the representative from Fire Department and the Professional Mechanical Engineer (PME) supervising the installation and test at least two sprinkler to be simulated for actual activation. Automatic sprinklers if required shall be in accordance with NFPA 13, Standard for the Installation of Sprinkler Systems. All automatic sprinkler and standpipe systems required by this code shall be inspected, tested, and maintained in accordance with NFPA 25, Standard for the Inspection, Testing, and Maintenance of Water-Based Fire Protection Systems.
Records of conducted maintenance and testing should be maintained. Sprinkler impairment procedures shall comply with NFPA 25, Standard for the Inspection, Testing, and Maintenance of Water-Based Fire Protection Systems.
A. Supervisory Signals
Where supervised automatic sprinkler systems are required by another Section of this Code, supervisory attachments shall be installed and monitored for integrity in accordance with NFPA 72, a distinctive supervisory signal shall be provided to indicate a condition that would impair the satisfactory operation of the sprinkler system. Monitoring shall include, but shall not be limited to, monitoring of control valves, fire pump power supplies and running conditions, water tank levels and temperatures, tank pressure, and air pressure on dry-pipe valves. Supervisory signals shall sound and shall be displayed either at a location within the protected building that is constantly attended by qualified personnel or at an approved, remotely located receiving facility.
B. Alarm Signal Transmission
Where supervision of automatic sprinkler systems is required, waterflow alarms shall be transmitted to an approved, proprietary alarm receiving facility, a remote station, a central station, or the fire station.
D. Valves.
Connections other than for fire service use shall be provided with an approved indicating-type valve and check valve located close to the supply such as tanks, pumps, and connections from waterworks system. Valves shall be provided to allow isolation of a standpipe without interrupting the supply to other standpipes from the same source of supply.
E. Pipe brackets and hangers.
Installation should be based on tables provided in the plans and specification in thickness, dimension and spacing for pipe support. Contractor shall provide safety precaution during work and shall be liable for the damages to person/property arising from the persecution of work
Supply of Fire Hose Cabinet with Glass & Complete Accessories (FHC)
Installation of Fire Hose Cabinet, surface mounted with brass nozzle fog adjustable, 1 A1/2- dia. X 30 meters long double jacketed with chrome pin lug coupling threaded, pull push mounting door with trim for all glass shall be guage # 18 wi note -œBRAKE GLASS IN CASE OF FIRE-, with 10 lbs or 4.5 kg. dry chemical type ABC portable Fire extinguishers with dimension of 750mm x 800mm, pipe connection should be 40mm diameter.
All one hundred millimeters (100 mm) dry standpipes shall be equipped with a four-way fire service connection. All fire service connections shall be located on a street front, not less than forty six centimeters (46 mm) or more than one hundred twenty two millimeters (122 mm) above grade and shall be equipped with an approved straight way check valve and substantial plugs or caps. All fire service connections shall be protected against mechanical injury and shall be visible and accessible. More than one fire service connection may be required.
A. - Inspector Test Connection should be installed with sprinkler heads for testing purposes by the Fire Department.
B. - Valves.
