Tenders are invited for Supply of ECG Products.

Any other such related suppliers or services as may be required for effective functioning of the Polyclinic.
All applicants are required to pick up a registration form at a non-refundable fee of GHc100.00 at the Procurement Unit and provide the following:
o A brief introductory letter indicating location, address, contact number/person and areas of specialisation/interest.
o Valid Business Registration Certificate and Certificate to Commence Business
o Valid GRA Tax Clearance Certificate (Evidence of Exemption if applicable)
o Valid Pharmacy Council Certificate (if applicable)
o Food and Drugs Authority Certificate (if applicable)
o Valid SSNIT Clearance Certificate
o Valid Works and Housing Certificate (if applicable)
o Any other relevant certificate/document should be made available
