Overview of Breakthrough ACTION Liberia
Breakthrough ACTION is a global SBC project funded by USAID to lead social and behavior change (SBC)
Breakthrough ACTION Liberia is the United States Agency for International Development (USAID)
flagship integrated social and behavior change (SBC) project. The project implementation period is
from February 2020 to April 2022. The project is led by Johns Hopkins Center for Communication
Programs in partnership with Save the Children, ThinkPlace, and ideas42.
The project works collaboratively with the Ministry of Health (MOH) programs at the national and
subnational level, and in complementarity to relevant USAID implementing partners (IPs) to improve
the effectiveness of SBC to achieve better, sustainable health outcomes. This activity contributes to the
Mission Country Development Cooperation Strategy (CDCS) Development Objective 3 of “Foundations
for growth strengthened through a healthy, productive, and educated population”.
Breakthrough ACTION Liberia is implemented in 12 counties to deliver effective, quality SBC activities
that will result in improved demand, access, and use of health services. To achieve this goal, the three
behavioral objectives are:
● Behavioral Objective 1: Significantly influence high impact behaviors related to malaria,
maternal, newborn, and child health (MNCH), family planning and reproductive health (FP/RH),
adolescent health, nutrition, prevention of infectious diseases, and water, sanitation, and
hygiene (WASH).
● Behavioral Objective 2: Develop the SBC capacity in Liberia for innovative behavior change
interventions at scale.
● Behavioral Objective 3: Strengthen SBC implementation across different thematic areas for
improved health outcomes.
Background
As of 2017, 38% of rural Liberians and 16% of urban Liberians lack access to a basic water service.12 The
proportion of households without basic water service has increased since 2001 in both rural and urban
settings. Water service access varies greatly by wealth quintile in both urban and rural contexts. In rural
settings, 36% of the poorest households continue to use surface water for drinking, compared to only
6% of the poorest households in urban settings.3 Despite the availability of water purification products
(most commonly bleach or WaterGuard), water treatment practices in Liberia remain low.4 According
to the Liberia DHS 2013, 14% of households appropriately treated their drinking water, with 10% using

collection time is not more than 30 minutes for a roundtrip including queuing

bleach or chlorine, and 4% using WaterGuard.5 These findings show a slight deterioration compared to
those reported in the previous DHS wave (in 2007), in which 16% of households used an appropriate
method to treat their drinking water nationally.6
Study Purpose and Objectives
Breakthrough ACTION Liberia plans to explore a comprehensive set of behavioral determinants related
to rural household sanitation and peri-urban household water practices through a multi-phased
formative research study to inform planned USAID WASH activities. The division of this research into
two phases will allow our research team to quickly deliver findings from the prioritized research
questions (rural sanitation) before moving on to focus on secondary research priorities (peri-urban
water). To guide this research, Breakthrough ACTION is using the socio-ecological model depicted in
Figure 1.
