Request for proposals are invited for Provision of Staff Medical Insurance Packages for Malaria Consortiun South sudan national Staffs
A. Malaria Consortium Malaria Consortium is one of the world's leading non-profit organizations dedicated to the comprehensive control of malaria and other communicable diseases in Africa and Southeast Asia. Malaria Consortium works with communities, government and non-government agencies, academic institutions, and local and international organizations, to ensure good evidence supports delivery of effective services, providing technical support for monitoring and evaluation of programs and activities for evidence-based decision-making and strategic planning. The organization works to improve not only the health of the individual, but also the capacity of national health systems, which helps relieve poverty and support improved economic prosperity.
The submitted proposal shall include the following information. Failure to supply all requested
information or comply with the specified formats will disqualify the bidder from consideration.
i) Name of the company, names of the authorized representatives and full contacts.
ii) Complete schedule of benefits indicating annual and lifetime benefits per participant based
on the technical requirements outlined in section C.
iii) Copy of business registration documents (proof of legal operation in South Sudan).
iv) Copy of license to provide health insurance in South Sudan
v) Bank financial Statement for 2018 (letter of sound financial standing)
vi) Detailsthat demonstrate their professionalskills, personnel and technicalresources. Thisincludes
information on corporate History including number of years offering similar services in South Sudan,
ownership, Management and technical qualification ofthe staff, with CVs of key staff included.
vii) References of at least 3 international NGOs with whom offer similar services, with permission for
Malaria Consortium to contact the references for feedback on the services
viii) Details of affiliated service centres/ health providerswho provide some or all of the services outlined in section C,
and who will accept the providers health insurance in the locations defined in B1 above.
ix) Details of which services as per section C are available through affiliated service centres/ health providers in each
locations as defined in section B1 above
x) Per location details of if the affiliated service centres/ health providers are direct pay or on a
reimbursement basis
xi) Details of how an evacuation process would work in situations of accidents and seriousillnessesthat
require specializedmedical attention.
xii) Details of the claims process and timelines for processing claims
