REQUEST FOR PROPOSAL Procurement of services for development of digital online forms for the Health Facility Assessment (HFA), installation onto tablets, training of users, cloud web-based hosting and management of data and provision of technical support during the assessment Procurement No. GF/RFP/23/053 UNDP Country Office Sudan is supporting the Federal Ministry of Health Sudan to implement a Health Facility Assessment with funding from the Global Fund to fight AIDS, Tuberculosis and Malaria. UNDP Country Office Sudan would like to hire a National ICT Consultancy firm to develop digital online forms for the Health Facility Assessment (HFA) based on WHO standard Health Facility Assessment Forms, configure and install the digital forms into tablets, training of technical and functional users of the form, establish a cloud web-based server to host all the data entered into the forms, cloud web-based hosting and management of data, and provide of technical support during the assessment period.
