1. The subject of the order is the supply of various medicines, including program medicines for the Provincial Hospital. John Paul II in Bełchatów. 2. Code and name according to the Common Procurement Vocabulary (CPV): 33.60.00.00-6 Pharmaceutical products 3. Description, size and scope subject of the order: 1) The Ordering Party divided the subject of the order into 58 parts (Packages No. 1 - 58), in accordance with the description included in Chapter XIII of the SWZ. 2) Other requirements regarding the execution of the order and principles of cooperation between the ordering party and the contractor is determined by the draft provisions of the public procurement contract, constituting Annex No. 3 to the SWZ. 3) If the description of the subject of the contract contains indications of trademarks, patents or origin, source or specific process that characterizes the products supplied by a particular economic operator, such an indication should be assumed to be accompanied by the words "or equivalent". In such a case, the ordering party allows the contractor to submit an offer for an equivalent preparation, such an offer consists in offering preparations identical to the subject of the order, for which there is a common name according to the index of international names. If the ordering party has any doubts regarding the equivalence of the offered drugs, the contractor will be obliged to provide explanations and demonstrate their equivalence at each request of the ordering party.
