CPV Description: Open procedure for the assignment of the urgent transport service with fixed wing aircraft of medical personnel for the removal of human organs for transplantation for a period of one year which may be renewable from year to year for a further 2 years.

Air transport services.

Urgent transport service with fixed wing aircraft of medical blood sampling teams for the purpose of transplantationfor a period of one year possibly renewable from year to year for a further 2 years.

The offer, pursuant to art. n. 93, paragraph 1, of Legislative Decree no. 50/2016 and accompanied by a guarantee equal to 2% of the total net amount placed on the basis of the tender (€ 6 400.00), subject to the reductions provided for by art. 93, paragraph 7, of Legislative Decree no. 50/2016. To guarantee the exact observance of the foreseen obligations-in the present specifications, a security equal to 10% of the contract amount will be required by law, in accordance with art. 103, paragraph 1 of Legislative Decree no. 50/2016

The subjects referred to in art. 45 of Legislative Decree no. 50/16;

Substitutive declaration, pursuant to the D.P.R. 28.12.2000 n. 445, certifying the possession of the COA (Certificate-Air Operator);

Substitutive declaration, pursuant to the D.P.R. 28.12.2000 n. 445, that is, for competitors not resident in Italy, equivalent suitable documentation made according to the legislation of the country of origin, certifying the absence of grounds for exclusion provided for by art. 80 of Legislative Decree no. 50/2016;

Substitutive declaration, pursuant to the D.P.R. 28.12.2000 n. 445, attesting-the inscription in the Register of the C.C.I.A.A. for the activity object of the contract;

Declaration of substitutive certification (ex D.P.R.445 / 2000) of not being in the interdictory cause to contract with the P.A. pursuant to art. 53, paragraph 16 ter of Legislative Decree no. 165/01;

- Substitutive declaration of certification (DPR 445/00) in which the participant declares to comply with the provisions of art. n. 3, paragraph 8 of Law 136/2010 (obligation of traceability of financial flows)
