The description of the subject of the order is included in the form Required parameters (attachment 2 to SWZ, hereinafter also referred to as FPW), this form should absolutely be submitted together with the offer form (attachment 1 to SWZ, hereinafter also referred to as FO).3. The Ordering Party requires that the offered equipment constitutes a medical product within the meaning of the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws of 2024, item 1620, as amended, hereinafter referred to as: UoWM), placed on the market and used in the territory of the Republic of Poland in accordance with applicable regulations.
