The Department of Veterans Affairs, Network Contracting Office 19, 6162 S. Willow Drive, Suite 300, Greenwood Village, CO 80111-5152 is soliciting quotations for Service Disabled Veteran Owned Small Business total set-aside for Water System Chemical Treatment Services for the VA Salt Lake City Health Care System (SLCHCS) in Salt Lake City, UT accordance with the Statement of Work (SOW). Please review all attachments for this requirement.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations and technical statements/documents are being requested, and a written solicitation document will not be issued. The Government reserves the right to make no award from this solicitation. This solicitation is a request for quotations (RFQ). The government anticipates awarding a firm fixed priced Base Plus 4 Option Years service contract.Special Standards of Responsibility To assist the contracting officer in determining if the special standards of responsibility applicable to this procurement are met, offerors are asked to provide the following information with their offers. Failure to include this information could result in a contracting officer s determination that an otherwise successful offeror is not eligible for contract award.SP1: Special Formal Degree & Certifications.
