2. Contract duration shall be for a period of three hundred (300) calendar days.
3. Price validity shall be for a minimum of forty-five (45) calendar days.
4. These documents shall be attached upon submission of the quotation: (1) PHILGEPS Registration Certificate, (2) Valid Mayor-s/Business Permit (3) Original Notarized Omnibus Sworn Statement (ABC-s above 50k), (4) Business/ Income Tax Return (for ABC-s above 500k).
