This RFQ is an invitation to submit offers for the provision of four (4) types of new Ultrasound Examination Training Phantoms. The requested items include eight (8) units each of the following: Obstetric Phantom, Female Pelvic Phantom, Scrotal Phantom, and Elastography Breast Phantom. All units areto be delivered to RDP in Red Deer, AB. The phantoms will be used by the School of Health and Wellness in the Diagnostic Medical Sonography Diploma Program. The selected Respondent will also be required to provide training to selected staff and faculty members (preferably in person) for the use ofthe Phantoms, inclusive of basic repair and support services for the Phantoms. It is RDP's intention to enter into the Form of Agreement attached as Appendix A to this RFQ with only one (1) legal entity. Mandatory Requirements: Respondents who do not comply with the following mandatory criteria willbe disqualified without further consideration given. Each proposal MUST include completion of the: Submission Form (Attachment #1), completed and signed by an authorized representative of therespondent.
