worksafebc (the worker`s compensation board) would like to identify providers interested in qualifying for the provision of community brain injury services to injured workers in and around the following under-serviced locations for specific postal codes as listed:

. Eastern british columbia: - postal codes v0b, v1n, v1r, v1c, v1a, v0a, v0g;

. Northern british columbia: - postal codes v0j, v1g, v2n, v2g, v2k.

if you are interested in providing these services for worksafebc, you are requested to complete and return the attached eoiq response form - appendices a, b and c. Worksafebc will continue to accept new submissions for this expression of interest and qualification document indefinitely until worksafebc chooses to conclude this process.

current contracted community brain injury services providers who responded to the previous rfq #028-2014 and eoiq #049-2014 and qualified to provide services to worksafebc, are not required to complete this new expression of interest and qualification document unless you are responding for a different/additional service location(s) than you are currently contracted for.
