Description : To Fabricate, Print, and Mail Out Juror Questionnaires and Summons Statewide for the State of Hawaii, The Judiciary. Work included in this contract shall consist of fabricating, printing, personalizing and mailing out Juror Questionnaires and Juror Summons with valid addresses. The Offeror shall be responsible for all costs for labor, tools, equipment and other appurtenances necessary to provide such forms and services. Services shall be provided for a (12) twelve-month period beginning July 1, 2024 through June 30, 2025, with the option to extend for three additional (12) twelve-month periods.
