Description : ONE JOB INSTALLATION OF TERMITE COLONY ELIMINATION/CONTROL SYSTEM FOR SURROUNDINGS AREA/PEREMETER OF NEW DOH WV CHD MAIN BUILDING AND WATER REFERENCE LABORATORY BUILDING
General Conditions:
a. Termite Elimination/Control system must be "Bait Type" (no chemical application)
b. Installation In-Ground (IG) bait system stations on the perimeter of DOH WV CHD
Main Building and Water Reference Laboratory bldg. Above-Ground (AG) system
is to be installed for indoor infestation if the need arises.
c. Conduct of monthly Monitoring of the bait station to cover one
(1) year starting upon completion of the installation of the stations.
d. Service Provider must be Licensed Pest Control Operator (PCO) by the Fertilizer
and Pesticide Authority with Proof of membership to various Pest Control Association.
e. Service Provider must be a duly certified "baited Type" Termite Colony elimination/
control system and is the authorize service provider for Western Visayas
f. Service Provider to Summit monthly report of monitoring/treatment Done.
