Description
This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 95-13 Personal services Contracts.
This synopsis/solicitation notice is a request for quote for a Non-Personal Service Contract for a Contractor to provide Telemedicine/On-site/Asynchronous Subspecialist healthcare providers specializing in various Sub-Specialty Clinic Services such as, but not limited to, Rheumatology (adults and pediatrics), Dermatology, Cardiology, Orthopedics, Developmental Pediatrics, and Gender clinic, etc., to the Chinle Service Unit (CSU), which consists of Chinle Comprehensive Health Care Facility, Pinon Health Center, Tsaile Health Center, Navajo Area Indian Health Service (IHS) throughout the duration of the contract period of performance. Services shall be performed in accordance with the attached Performance Work Statement (PWS). The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment. This notice is for multiple positions.
PERIOD OF PERFORMANCE: The Government intends to award a labor hour type contract consisting of (1) one-year base period plus (3) one-year option periods. Tentative start date: 7/01/2024
