c. PhilGEPS Registration Number;
d. Valid Tax Clearance (per Executive Order 398, Series of 2005, as finally reviewed and approved by the BIR)
II. Original Copy of:
e. Conformity with the Technical Specifications (Attached as Annex A)
f. Financial Proposal Form (Attached as Annex B)
g. Notarized Omnibus Sworn Statement (Attached as Annex C)
