Please accomplish the attached Request for Quotations/Proposal (RFQ/P) Form for the SUPPLY AND DELIVERY OF PLASMA STERILIZATION CONSUMABLES (ETHYLENE OxIDE, CHEMICAL INDICATOR STRIP, etc.) and indicate your confirmation on the Terms and Conditions by signing the Undertaking. Kindly submit the accomplished SEALED PROPOSAL together with the following documentary requirements for Small Value Procurement (SVP):
- Valid Mayor-s / Business Permit;
- PhilGEPS Registration Number / Certificate;
- Notarized Omnibus Sworn Statement (Original copy)
- Income Tax Return (ITR)
