Signed SF 14492. Completed price schedule for all LINS & completed attachment D.2 Supply Line-Item Template.3. Signed SF30s for any amendments issued4. Vendor shall be an OEM, authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM.5. Proposals shall clearly articulate how the vendor s product fulfills the requirements listed in section B.2 Statement of Work and providing a completed attachment D.1 Evaluated Salient Characteristics spreadsheet.7. Completed copy of VA Form 6550 Pre-Procurement Assessment.8. Specification/cut sheets for any/all network and hardware components.9. The Contractor shall adhere to VA national site-to-site VPN or work with the VA Office of Cyber and Information Security and VISN 23 Information System Security Officers to establish a client-based VPN.10.
