Signed SF 14492. Completed price schedule for all LINS & completed attachment D.2 Supply Line-Item Template.3. Signed SF30s for any amendments issued4. Vendor shall be an OEM, authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM.5.
