Indonesia continues to face a significant tuberculosis (TB) burden, with more than one million estimated cases and over 130, 000 TB-related deaths annually. According to the Global Tuberculosis Report 2024, only 74% of TB cases in Indonesia are notified, meaning a substantial number go undiagnosed or untreated. This gap contributes to continued transmission, poor treatment outcomes, and rising drug resistance.Early detection is a cornerstone of TB control. Timely diagnosis reduces transmission, enables more effective treatment, and prevents complications. Integrating TB screening into the Annual Free Health Screening Program (Cek Kesehatan Gratis), particularly through CKG Ulang Tahun, presents a timely and strategic opportunity to close the detection gap and reach underserved populations.To support this effort, the National Tuberculosis Program (NTP) will pilot a “one-stop TB service” in 46 Puskesmas as part of the CKG Ulang Tahun in Q4 2025. This pilot will include chest x-ray screening and the use of a point-of-care molecular diagnostic test (Pluslife), enabling same-day diagnosis and initiation of treatment. This approach is designed to reduce delays in care and improve treatment outcomes by streamlining the TB care pathway.To inform the design and implementation of this pilot, the World Bank will support NTP in conducting a small-scale trial (pre pilot) activity in 5 puskesmas (Sites to be determined - West Java or Central Java). This activity will help assess feasibility, generate operational insights, and ensure readiness for the wider pilot implementation.
