Tender Details

1. The subject of the order is the supply of catheters for intravascular imaging along with the lease of devices, hereinafter referred to as medical devices, to the pharmacy's warehouse,

2. The type and quantity of disposable equipment are specified in the assortment and price forms constituting Annexes 1.1-1.3 to SWZ.

3.TerThe minimum validity period of the offered medical devices cannot be shorter than 12 months from the date of delivery.

4. The contractor is obliged to deliver the subject of the order together with information (leaflet) in accordance with applicable law.
