The subject of the order is the supply of reagents and consumables, together with the rental of an analyzer for testing glycated hemoglobin using the HPLC method at the Ordering Party's headquarters (Kraków, os. Zotej Jesieni 1), in accordance with the description and requirements contained in Annex No. 1A to the SWZ and the limit parameters regarding the rental of the analyzer included in Annex No. 1B to the SWZ. 1. Additional conditions regarding the subject of the order: a) the expiry date of the reagents cannot be shorter than 6 months from the date of delivery, b) the warranty period of the devices: not shorter than duration of the lease agreement, i.e. 36 months, c) requirements regarding device servicing: service response time to reporting a failure: 24 h from the date and time of telephone or written (including e-mail or fax) notification by the Ordering Party, telephone contact with The Ordering Party, in the event of a device failure that cannot be removed within 48 hours from the date and time of the telephone or written (e-mail or fax) notification by the Ordering Party, the Contractor will be obliged to provide the Ordering Party with a replacement device on time. repair of the rented device, with parameters no worse than those specified in Annex 1, or cover the costs of...
