1. Name of the order: SUPPLY OF PHARMACEUTICAL PRODUCTS for the Provincial Specialist Hospital. The Blessed Virgin Mary in Częstochowa. 2. The subject of the order includes 75 parts, i.e.: Parts No. 1-13 - PROGRAMME DRUGS, Parts No. 14-62 - DRUGS BASIC, Parts No. 68-70 - ANTIBIOTICS, Parts No. 63-67 - NARCOTIC AND PSYCHOTROPIC DRUGS, Part No. 71 - DIAGNOSTIC FACTOR Part No. 72-73 - DIALYSIS FLUIDS Part No. 74 - FOOD FOR SPECIAL MEDICAL PURPOSES, Parts No. 75 - RADIOPHARMACEUTICAL 3. The detailed scope of the order was specified in Annex No. 2 to the SWZ - assortment and price form. Other conditions regarding the implementation of the order are specified in the draft provisions of the contract (PPU), constituting: Annex No. 6 to the SWZ. 4. All requirements specified in the documents indicated above are minimum requirements and their fulfillment is obligatory. Failure to meet the above-mentioned minimum requirements will result in rejection of the offer as inconsistent with the conditions orders pursuant to Art. 226 section 1 point 5 of the Public Procurement Law. 5. The Ordering Party allows the submission of partial offers. The concept of a partial offer means individual parts specified in Annex No. 2 to the SWZ.
