1. The subject of the contract is the supply of medical equipment and apparatus (intensive care beds, hypothermia set, bronchoscopy set). The subject of the contract has been divided into 3 parts.
2. A detailed description of the subject of the order, specifying the requirements of the Ordering Party and the quantity, is includedin Appendix No. 1A to the SWZ - Price form, Appendix No. 1B to the SWZ - Description of the Subject of the Order and in Appendix No. 5A and 5B to the SWZ - Draft contracts. By completing the items indicated in Appendix No. 1B to the SWZ, the Contractor confirms that the requirements specified by the Employer are met.
3. The subject of the contract must comply withthe Act of April 7, 2022 on medical devices (i.e. Journal of Laws 2022.974 of May 2022, 2022), if the Ordering Party requires the offer of an assortment constituting a medical device.
4. The purchase of medical equipment and apparatus is carried out under the National Transplantation Program - regarding part no. 1, cpart 2 task 1 and part 3.
