Lazaro at the Office of the Procurement Division on February 4, 2019 @ 2:00 pm, afterwhich it shall be opened.

NOTES:
1. Quotation may be submitted either typewritten (preferred) or handwritten.
2. ALTERNATIVE BIDS/A BID WITH OPTIONS SHALL BE REJECTED.
3. Request For Quotation can be obtained from the Office of Procurement Division on January 31 to February 4, 2019.
4. Warranty shall be for a period of one (1) year, which shall commence after the final acceptance of the end-user
5. Price validity shall be for a period of 120 calendar days from opening of bid.
6. Submit sample/s and brochure/s, if applicable, showing specifications of product being offered. Likewise state the brand in the RFQ or bid proposal.
7. If awarded, the bidder shall submit the required Registration Documents.

SCOPE OF WORK:
1. Conduct inspection on the actual condition of the transformer, record name plate,
date and other information.
2. Supply and installation of electrical materials listed above.
3. Conduct standard testing of dry type transformer.
a. Transformer turn ratio.
b. Winding resistance test.
c. Excitation current test.
d. Ground earth resistance test.
e. Polarization index test.
f. Insulation resistance test.
4. Conduct cleaning and retightening of terminals of Transformer No. 9
5. Energization/commissioning.
6. Submission of test report.
7. Cleaning of work site.
8. It includes electrical works not stated above but are required and necessary for the
intent completion of the project.
Notes:
1. The bidders must have PCAB license of at least Category C & D, registration particular small B in electrical works.
2. The bidders must state the brand and technical specifications of offered items.
3. The bidders must submit valid certificate of calibration for the test instrument that will be used for testing.
