Tender Details

1. The subject of the order is the supply of heart support devices for the Hospital of the Ministry of Internal Affairs and Administration in Rzeszow, divided into 10 tasks. 1.1. Detailed description of the subject of the order - a list of required technical parameters is attached as Annex 4 to the specifications of the terms of the order. 1.2. Subject of the order was divided into 10 tasks (parts). 1.3. Deliveries of the subject of the order will take place on the basis of an "equipment bank", i.e. the Contractor is obliged to deliver equipment based on quantity and assortment orders placed by the Ordering Party until the given assortment is exhausted. The Supplier will deliver ordered equipment for the Warehouse of the Electrophysiology and Electrotherapy Laboratory located in the main building of the Hospital (building "A). Medical materials (after prior arrangement with the Ordering Party) will be invoiced only after the invasive procedure has been performed. Inventories in the "equipment bank" remain the property of Contractors. The document of acceptance into the "equipment bank" is the transfer protocol. 1.4. The subject of the order also includes the delivery of the subject of the order by the contractor's transport, at his expense and risk. 1.5. The Ordering Party allows the submission of partial offers in accordance with tasks 1 - 10. 1.6. The contractor may submit offer for any number of tasks.
