Legionów 10, 41-902 Bytom. 2. The subject is implemented as part of the Project KPOD.07.03-IP.10-0419/25 entitled Accelerating the processes of digital transformation of health care through the development of digital services at the Provincial Specialist Hospital No. 4 in Bytom" as part of the investment D1.1.2 "Accelerating the processes of digital transformation of health care through further development of digital services in health care. 3. A detailed description of the subject of the order is set out in Annex No. 1A - Assortment and price form, Annex No. 1B - Description of the subject of the order (in the scope of Part/Package No. 1-2).
