REQUEST FOR QUOTATION

Victorias City Water District (VCWD), will undertake a Small Value Procurement for the Supply & Delivery of Water Bills Official Receipt in accordance with Section 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184.

Delivery Terms : Thirty (30) days from the receipt of the Notice to Proceed

Interested suppliers are required to submit their valid and current SEC Registration Certificate, DTI Registration Certificate, Mayor-s Permit, Income/ Business Tax Return, PhilGEPS Registration Certificate, Price Quotation Form (Annex -œA-), Statement of Compliance, Schedule of Requirements, Notarized Omnibus Sworn Statement accompanied by the duly notarized Special Power of Attorney, Board/Partnership Resolution or Secretary-s Certificate, whichever is applicable.

Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above and other terms and conditions stated in the price quotation form.

Any interlineations, erasures or overwriting shall be valid only if they are signed or initialed by the bidder or his/her duly authorized representative/s.

Submission of the quotation and eligibility documents is on or before 12:00 P.M. of August 22, 2018 at the Victorias City Water District. Place all documents in a properly labeled and sealed envelope.
