Description : PLEASE REFER TO THE ATTACHED PURCHASE REQUEST (PR), REQUEST FOR QUOTATION (RFQ) & TERMS OF REFERENCE (TOR) FOR THE ABC, TEMPLATE FOR BID PROPOSAL AND TECHNICAL SPECIFICATIONS, RESPECTIVELY.
The prospective bidder shall submit the following eligibility documents as prescribed below:
a. SEC or DTI Registration Certificate;
b. Mayor-s Permit;
c. Current Tax Clearance Certificate;
d. PhilGEPS Registration;
e. Prescribed Omnibus Sworn Statement (notarized, for Php50K and above ABC); and
f. Current Income Tax Return duly received by BIR (for Php500K and above ABC)
Financial Proposal and complete set of eligibility documents shall be submitted in either of the following manner.
a. Manual/physical submission in a sealed envelope at the PCA Regions I, II, and CAR - Regional Office, Brgy. Tomana West, Rosales Pangasinan; or
