Please quote your best quotation on the line item(s) in the attached Quotation Form, subject to the attached Terms and conditions in the associated documents. Copies of the following eligibility, Technical & Financial requirements are also required to be submitted along with your quotation/proposal:



Technical Documents:

1. Current Valid Mayors Permit or Business Permit

2. BIR/DTI Registration Certificate

3. Sworn affidavit of the bidder that it is not related to the head of the procuring entity, members of the BAC, TWG and Secretariat and members of the PMO or end-user Unit by consanguinity or affinity up to the third civil degree.

The Local Government of Busuanga reserves the right to reject any or all the bids, waive any defect or informality therein, accept the bid and award the contract to the most advantageous offer to the local Government of Busuanga, for and in behalf of the project. Local Government of Busuanga assumes no responsibility to compensate or indemnify the bidder for any expense or loss that may be incurred in the preparation of the bid.
