REQUEST FOR QUOTATION OF PRICES

The Local Government of Pres. M. A. Roxas, Zambo.Norte is inviting duly licensed supplier/distributor,

to submit price quotations for the suppy and delivery of OFFICE SUPPLIES_MMO supplies not later

t h a n 2 : 0 0 in the a f t e r n o o n o n OCTOBER 25, 2018 at which time and date the

same shall be opened in the presence of the members of the Bids and Awards Committee or their duly

authorized representatives.

To be eligible to participate, interested parties are required to submit the following documents as

prescribed under R.A. 9184, otherwise known as the Government Procurement Act, viz :

a. Department of Trade and Industry (DTI) business name registration or SEC registration

certificate whichever maybe appropriate under existing laws of the Philippines;

b. Valid and current Mayor's Permit/Municipal License;

c. Taxpayer's Identification Number

Suppliers already accredited need not to submit the above requirements.

Eligible suppliers shall submit their price quotations through their authorized managing officer or

duly authorized representative in the prescribed form, including its annexes, if any on or before the

specified deadline in sealed envelope, which shall be addressed to the Bids and Awards Committee,

Pres.M.A. Roxas, Zambo. del Norte and marked "DO NOT OPEN BEFORE 2:00 P.M. OCTOBER 25, 2018

Bids submitted after the deadline shall not be accepted by the B A C.

All price quotations shall be inclusive of fees, taxes and imposts of the foods listed at the back

thereof, which the Province of Zamboanga del Norte will buy from you if the prices and terms are found

to be advantageous of the government. Bid form payment in the amount of P250.00 will be

collected per BAC Resolution No. 25, series of 2011 for the sale and prescribing of prices of bidding .

documents and other fees.

The Local Government Unit of Pres. M. A. Roxas reserves the right to reject any or all bids, declare

a failure of bidding or not to award the contract, and to accept such bid as maybe found to the

advantageous to the government.
