REQUEST FOR QUOTATION
1. The Philippine Science High School Western Visayas Campus (PSHSWVC) through the General Appropriations Act (GAA) for CY 2019, will undertake a Small Value Procurement for the -œSupply, Delivery, Installation, Configuration and Testing of PSHS-WVC WiFi Access Points.- (1 LOT)in accordance with Section 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184.
Name of Project : Supply, Delivery, Installation, Configuration and Testing of PSHS-WVC WiFi Access Points.
Approved Budget for : One Hundred Thirty Two Thousand Eight Hundred Pesos Only
Contract (PhP132, 800.00)
Location : PSHSWVC, Iloilo City
Delivery Term : Within sixty (60) calendar days upon receipt of Approved Purchase Order
2. Interested Suppliers are required to submit their valid and current or latest Mayor-s/ Business Permit, Income/Business Tax Return for, PhilGeps Certificate of Registration, Omnibus Sworn Statement and Request for Quotation Form (Annex -œB-) during submission of offer/quotation.
3. Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above and other terms and conditions stated in the Request for Quotation form.
4. Any alterations, erasures or overwriting shall be valid only if they are signed or initialed by the bidder or his/her duly authorized representative/s.
5. Submission of quotation and eligibility documents is on or before 3:00 p.m. of December 19, 2019 at the PSHSWVC, Bito-on, Jaro, Iloilo City, Old Academic Building, 1st floor, Procurement Office. Open submission may be submitted, manually, email or through facsimile at fax no. 033-3295644.
