Submission of a written request to the Director General, Securities and Exchange Commission of Sri Lanka, Level 29, East Tower, World Trade Centre, Echelon Square, Colombo 01, Sri Lanka, along with a payment of a non-refundable fee of USD 40 or LKR 12, 500/-
