LOCATION/DURATION:

Duration: 90 days over 6 months

Location: Two trips of 3-4 weeks each to Papua New Guinea and remaining time home-based.

Reporting to: Immunization Specialist, UNICEF PNG; with technical support from Monitoring Specialist, Director-s Office, UNICEF Supply Division, Copenhagen

BACKGROUND:

In 2015, Gavi, the Vaccine Alliance, approved a 2016-2020 strategy to support countries in improving their immunization supply chains. The strategy includes five areas: human resources & leadership, improvement planning, data for management, cold chain equipment and system design. Gavi undertakes a number of measures to support the implementation of the 2016-2020 strategy including country grants and programme funding, secretariat engagement and Partners' Engagement Framework (PEF). PEF includes targeted country assistance and under this engagement, UNICEF supports Papua New Guinea (PNG) in its efforts to improve data use and human capacity throughout the immunization supply chain. The Government of Papua New Guinea (GoPNG) has committed to reforming the Health Medical Supplies Procurement and Distribution including the decentralization to the provincial health authorities. This reform is being supported by organizations such as Asian Development Bank, Australian Aid (Department of Foreign Affairs and Trade), Global Fund (GF), Gavi, WHO and UNICEF amongst others and the focus on the reform includes focus on human resource capacity and better data use.

The Data for Management (D4M) initiative is a key strategy to support countries enhance the use of data to oversee and manage key aspects of the immunisation supply chain in all Gavi countries by 2020. The D4M aims to establish a data-driven culture at all levels of the national immunization supply chain through strengthened availability, use and understanding of the value of data to manage and improve the performance of the supply chain.

In 2016, Effective Vaccine Management (EVM) assessment showed some weaknesses in the management of the vaccines in PNG, such as lack of relevant trainings, standard operating procedures, recording and use of essential immunization supply chain data.

In April 2019, an Immunization Supply Chain Data and HR Assessment was conducted. The assessment showed that one of the main reasons for poor vaccine demand planning at the national and provincial storage facilities in PNG is the lack of accurate data on which to base calculations and decisions. It showed that incomplete and inaccurate data leads to guesswork, which in turn leads to wastage and stockouts in the country. The same assessment narrated instances of late submissions of reports, incomplete and inaccurate stock levels indicated on vaccine order forms from health centres, and inconsistent and non-standardised stock keeping methods at provincial vaccine stores. It also highlighted a need to improve aspects of the existing paper-based ordering system between health centres and provincial offices.

Based on identified challenges and recommendations from the recent assessments, UNICEF is supporting PNG National Department of Health (NDOH) with implementation of solutions to improve the visibility and use of data in the short term.

The main work of this consultancy will be on the following areas:

· Strengthening the availability of quality immunisation supply chain data from current paper-based LMIS,

· Strengthening processes through developing/updating key standard operating procedures (SOPs)

· Encouraging data use through establishment of standard key performance indicators (KPIs) and visualizations, and

· Capacity building of health staff in data reporting and use through trainings and supervision.

The outputs of this consultancy will enable health workers and managers in PNG to have access to the essential data points needed for management of the immunization supply chain.

DESCRIPTION OF ASSIGNMENT:

The background for the work in this consultancy is the iSCM HR/Data Assessment report from April 2019. In addition, the consultant will visit a limited number of health facilities and provinces to gain the required detailed information on the data to update LMIS tools, develop SOPs, and propose KPIs and visualizations. The visualisation of data on KPIs agreed with NDOH will be based on standard dashboard developed jointly by WHO, UNICEF and Gavi. The field visits will be conducted in 1-2 weeks only. The consultant will prepare a short presentation with the key findings for each of the specific areas described below. This presentation will serve as the guide for the further implementation of the consultancy.

Strengthen LMIS data collection and reporting system for immunisation supply chain

Based on the results from the April 2019 Assessment and visits to the health facilities, the consultant will update and develop paper-based LMIS record and reporting formats for the immunization program to ensure relevant data can be simply and effectively recorded and reported to allow for supply chain management and performance monitoring. When amending and developing LMIS tools, the consultant will consider at which level the records and reports will be used and the information needed at the various levels for supply chain management.

Develop SOPs for detected gaps

The consultant will develop standard operating procedures (SOPs) identified in the Assessment and field visit. The focus on the SOPs is to strengthen the reporting and use of supply chain data. Below is a list of the proposed SOPs from the Assessment:

· Recording and Reporting Vaccine Temperatures

· Stock Count and Reporting of Vaccines and Injection Devices

· Updating the Cold Chain inventory

· Storage/warehouse management (record keeping, FEFO/FIFO)

· Release of commodities

· Handling of expired vaccines, medicines and supplies

· Filling and submitting supply chain reports

· Use data to manage the supply chain and track commodities

The final list of SOPs and the supply chain level they will be applicable to will be determined at the presentation of the field visit findings. As needed, the consultant will develop a short guide on the introduction of the SOPs in relevant facilities and warehouses.

Identify and support visualization of key performance indicators to monitor immunization supply chain performance

Considering the standard immunization supply chain indicators (www.technet-21.org/iscstrengthening) and based on available data at different supply chain levels, the consultant will identify relevant key performance indicators (KPIs) that can be used by managers to monitor and improve the immunization supply chain at different levels (National, Provincial and District). The consultant will detail the purpose, calculation and possible visualization of each KPI and discuss with GoPNG to reach agreement of the final KPIs. Based on the final KPI list, the consultant will propose how the KPIs should be used, including visualized, at each supply chain level.

Develop training materials and conduct relevant D4M trainings and strengthen supportive supervision

Based on the capacity gaps identified during the field visits, the consultant will develop a training module to introduce the new LMIS tools, SOPs, and KPIs. Upon GoPNG and UNICEF approval of the training material (including the training plan), the consultant will conduct and facilitate training of trainers session and support the trainers during a few selected trainings. The consultant will also identify how to strengthen the supervisions at different supply chain levels in the area of data reporting and use, and develop a check list that can be used for this purpose.

Support strengthening of the National Logistics Working Group (NLWG)

The consultant will work on endorsement and operationalization of the NLWG including through review of draft TOR. The consultant will develop a short training module for the NLWG members on supply and data for management best practices and facilitate the training.

KEY DELIVERABLES:

· Powerpoint presentation of the findings from the field visits, clearly showing the proposed specific deliverables for the consultancy

· Revised and/or new immunization LMIS records and reports recommended for use, must be of print-ready quality

· Visual SOPs for immunization supply chain, as per agreed list with GoPNG and UNICEF

· Concise report with recommendations on immunization supply chain indicators for each level with description of data sources and calculations, as well as benefits and challenges of implementation

· Proposal for a visualization tool of key performance indicators with consideration of already existing visualization work, need to be developed for each supply chain level

· Proposed strategies for strenghtening supportive supervision and development of supervision check list, with the focus on data reporting and use

· Training materials for strengthening reporting and data use on iSCM

· Training reports for the conducted trainings and updated training material as per learning and participants feedback

· Final TOR for NLWG

· Training module for NLWG members in data for management best practices.
