50 pax Lunch (12:00 NN): Assorted bread rolls and butter, cream of mushroom, Australian cobbler fish fillet with tomato coriander sauce, crispy thai chili pork ginger, pork rib with barbeque sauce, vegetable gratan, rice, assorted baked cheese cakes, glass of iced tea (8 oz.)

Approved Budget for the Contract: Ninety Five Thousand Pesos (P 95, 000.00)
Interested suppliers are required to submit the following in a sealed envelope:
1. Price Quotation;
2. Copy certified Mayor-s/Business Permit;
3. Copy certified PhilGEPS Registration Certificate/Number;
4. Copy certified Tax Clearance;
5. Income/Business Tax Return for last six (6) months prior to the submission and opening of quotations;
6. Omnibus Sworn Statement;
7. Authority of Representative/Signatory (in the form of a Special Power of Attorney for Single Proprietorship or Board Resolution/Secretary-s Certificate for Corporation); and
8. Photocopy of One (1) valid government ID, except Postal ID, of the Authorized Representative/Signatory.
Submission of quotation and other documentary requirements is on or before June 4, 2019, 10:00 a.m. at the Office of the BAC Secretariat, 7th Floor, Iloilo City Hall, Plaza Libertad, Iloilo City. Late submission shall not be accepted.
