5. Warranty shall be for a period of six(6) months for supplies and materials, one(1) year for equipment, from date of acceptance by the procuring entity;

6. Price validity shall be for a period of one hundred twenty (120) calendar days; and

7. The supplier with the lowest quotation shall submit the following documents, to wit:

a. SEC/DTI Registration

b. Business/Mayor-s Permit

c. BIR Registration Certificate

d. PhilGEPS Registration Certificate
