Description : INVITATION TO SUBMIT QUOTATION FOR THE PROCUREMENT OF GOODS RECOMMENDED FOR SMALL VALUE PROCUREMENT PURSUANT TO SECTION 53.9 OF THE REVISED IMPLEMENTING RULES AND REGULATIONS-A (IRR) OF REPUBLIC ACT (RA) 9184

Prospective Bidders shall submit the following requirements together with the accomplished RFQ:
-Valid PhiIGEPS Registration Number/Certificate;
-Business Registration issued by the Department of Trade of Industry (DTI), Security and Exchange Commission (SEC) or Cooperative Development Authority (CDA) whichever applicable (ONLY TO SUBMIT IF REGISTERED UNDER PHILGEPS RED);
-Valid Mayor's/Business Permit issued by the city or municipality where the principal place of business of the prospective supplier/service provider is located (ONLY TO SUBMIT IF REGISTERED UNDER PHILGEPS RED);
-Certificate of TAx Clearance issued by the BIR;
-Notarized Omnibus Sworn Statement (Revised);
-Company Profile; and
-Product Brochure with complete technical specifications of the offered product with photos.
