REQUEST TO SUBMIT BID OR PROPOSAL FOR SUPPLY AND DELIVERY OF VARIOUS OFFICE AND COMPUTER SUPPLIES FOR USE IN THE IMPLEMENTATION OF RICE PROGRAM, DA-RFO5, SAN AGUSTIN, PILI, CAMARINES SUR AT APPROVED BUDGET FOR THE CONTRACT AMOUNTING TO ONE HUNDRED TWENTY FIVE THOUSAND EIGHT HUNDRED EIGHTY TWO PESOS AND SEVENTY FIVE CENTAVOS (PHP125, 882.75) PURSUANT TO ALTERNATIVE METHOD OF PROCUREMENT SPECIFICALLY NEGOTIATED PROCUREMENT- SMALL VALUE PROCUREMENT UNDER SECTION 53.9 UNDER RA 9184 AND ITS REVISED IRR
The Department of Agriculture Regional Field Office No. 5 (DA RFO 5), San Agustin, Pili, Camarines Sur is inviting interested suppliers to submit complete bid proposal for Supply and delivery of various office and computer supplies for use in the implementation of Rice Program, DA-RFO5, San Agustin, Pili, Camarines Sur. The agency intends to apply the amount of One Hundred Twenty Five Thousand Eight Hundred Eighty Two Pesos and Seventy Five Centavos (Php125, 882.75) as the Approved Budget for the Contract.
As per approved Purchase Request and considering the nature of requisition being not more than Five Hundred Thousand Pesos (Php500, 000.00) and in order to fast-track the implementation of the program, this procurement is facilitated thru Negotiated Procurement-Small Value Procurement under Section 53.9 of RA 9184 and its Revised IRR.
As such, Bonings Trading, Naga City; JEB Trading, Naga City; Papel @ Pluma, Naga City; Copylandia Office System Corp, Naga City; Bitstop Bicol Sales Center, Naga City and 3Gx Computers & IT Solutions, Naga City are hereby requested to submit the following documents:
1. DTI Registration/SEC Registration
2. BIR Registration
3. 2016 Mayor-s Permit
4. Tax Clearance Certificate (for bidding purposes)
5. PhilGEPS Registration
6. Affidavit of Disclosure
The bidders are required to pay the non-refundable amount Php500.00 as Annual Accreditation fee (2016) and Php500.00 for bid documents to the Cashier-s Office, DA RFO-5, Pili, Camarines Sur. All submitted documents must be in duplicate, otherwise, bids will be automatically rejected on the ground of non-compliance with the prescribed bid requirements.
Sealed Bid must be received by BAC Secretariat, DA RFO 5 not later than 9:30 in the morning of 23 September 2016 at DA-LIBRARY, Research Building, DA RFO-V, Camarines Sur. Opening of proposal will be on the same date and time.
