Together with the quotation, the participating prospective suppliers shall also submit the following requirements:
1. Valid and current Mayor-s/Business Permit
2. PhilGEPS Certificate of Registration and Membership (PLATINUM/BLUE)
3. Tax Clearance
The Approved Budget for the Contract (ABC) is Two Hundred Sixty Five Thousand Eight Hundred Fifteen Pesos and Twenty Five Centavos (PhP265, 815.25) only.
Subject items must be completely delivered within thirty (30) days upon receipt of the Purchase Order by the supplier at BFAR-RO 5 Fabrica, Bula, Camarines Sur. The delivered items must be new and free from defects or damages, must be food and good grade. Items found defective during delivery shall be replaced immediately.
All quotations shall be submitted to the Office of the BAC Secretariat, BFAR 5, Fabrica, Bula, Camarines Sur before 10:00 o-clock in the morning of March 25, 2019, which shall be opened and evaluated by the BAC thereafter at the BFAR 5 Conference Room at the same address.
This requisition is made thru Alternative Mode of Procurement under Rule XVI of the 2016 Revised IRR of RA 9184, specifically thru Shopping under Section 52.1b and 52.2, as per prior recommendation of the Bids and Awards Committee.
