REQUEST FOR QUOTATION (RFQ)

The Department of Environment and Natural Resources-PENRO Bulacan, through its Management Services Division, will undertake a Small Value Procurement for the -œSupply and Delivery of Safety Equipment and Materials for the Conduct of Protected Area Suitability Assessment (PASA) of Angat Watershed Metro for the 3rdQuarter of CY 2019- in accordance with Section 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184.

Interested suppliers are required to submit their valid and current Mayor-s Permit, BIR TIN, SEC/DTI Registration, PhilGEPS Registration Certificate, and Price Quotation Form (Annex -œA-) during submission of offer/quotation.

Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above and other terms and conditions stated in the price quotation form.

Any interlineations, erasures or overwriting shall be valid only if they are signed or initialed by the bidder or his/her duly authorized representative/s.

Submission of quotation and eligibility documents is on or before 4:00pm of August 21, 2019 at the PBAC Office, PENRO Bulacan Building, Citta Di Oro Subdivision, Sta. Rita, Guiguinto, Bulacan.
