●Submit the latest (within the last six months Microbial Water Test Laboratory Certificate and Chemical and Physical Water Test Laboratory Certificate issued by a DOH accredited water-testing laboratory or the Food and Drug Administration, pursuant to DOH Administrative Order No. 2017-0010 dated June 23, 2017
