1. The Bangko Sentral ng Pilipinas requests quotations for the hereunder requirement:

Brief Description of the Requirement : Supply and Delivery of One (1) unit Hot Stick, as per BSP Technical Specifications and Terms and Conditions

Approved Budget for the Contract (ABC) :



PhP65, 000.00, VAT inclusive

(Quotations/Proposals exceeding the ABC shall be disqualified)

Delivery Period : Within thirty (30) calendar days from receipt of a copy of the executed Purchase Order

Product Warranty : Valid for one (1) year from delivery date

Deadline and Details of Submission of Financial Proposal : 14 May 2019, 12:00 NOON

2. All quotations/proposals that meet the deadline shall be opened by the SOMD-CMS Committee on Procurement at 2:30 P.M. on 14 May 2019. Late submission of quotations/proposals shall be considered as disqualified.



3. The Supplier with the Single or Lowest Calculated Quotation shall submit the following documents, within five (5) calendar days upon receipt of notice. Failure to do so shall result in the post-disqualification of its quotation/proposal:



A. LEGAL DOCUMENTS

a. Valid Y2019 Mayor-s/Business Permit, or the equivalent document for Exclusive Economic Zones or Areas.

b. Philippine Government Electronic Procurement System (PhilGEPS) Registration Number/Certificate.











B. TECHNICAL REQUIREMENTS

a. BSP Terms and Conditions, duly signed by the Supplier-s authorized representative on the Conforme page (1 page).

b. BSP Technical Specifications, duly initialed/signed by the supplier-s authorized representative (1 page).

c. BSP Supplied Evaluation Matrix (BSP-SEM), duly accomplished and initialed/signed on each and every page by the bidder-s authorized representative (3 pages).

d. Bidder-s proof of offer (containing all the technical information listed in the attached BSP-SEM and other relevant information) necessary for the evaluation, in any of the following:

i. Product Brochure(s) from the manufacturer [paper copy or downloaded from the internet with indication of its source (e.g., website)]; or

ii. Manufacturer-s Catalog highlighting the data conforming to the BSP specifications;

or

iii. Certification from the manufacturer that the product conforms to the specifications stated in the bidder-s duly accomplished BSPSEM; and

e. Omnibus Sworn Statement.



Note: Use BSP prescribed form. Omission of any of the provisions in the Omnibus Statement shall be considered as post-disqualified.



4. The BSP assumes no responsibility whatsoever to compensate or indemnify any supplier for expenses incurred in the preparation of quotations/proposals.



5. The BSP reserves the right to reject the Financial Proposal or not award the contract and makes no assurance that a Contract shall be entered into as a result of this project.
