INVITATION TO APPLY FOR ELIGIBILITY AND TO BID (IAEB)



1. The Wao District Hospital (WDH) through the 2019 Off-Budgetary Fund intends to apply the sum of Four Million Four Hundred Fourteen Thousand Two Hundred Fifty Pesos and 50/100 (P 4, 414, 250.50) being the Approved Budget for the Contract (ABC) to payment in the hereunder project, to wit:

2. The WDH now invites bids for the procurement of the above-captioned project. Delivery of the Goods is required within ten (10) calendar days from receipt of the Notice to Proceed (NTP). Bidders must have completed, within five (5) years from the date of submission and receipt of bids, at least two (2) contracts similar to this Project and the aggregate contract amounts should be equivalent to at least fifty percent (50%) of the ABC.



The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders.



3. Bidding will be conducted through open competitive bidding procedures using a non-discretionary -œpass/fail- criterion as specified in the Implementing Rules and Regulations (IRR) of Republic Act (RA) 9184, otherwise known as the -œGovernment Procurement Reform Act-. Bidding on this Project will be on a -œper item- basis.



Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to Filipino citizens, pursuant to RA 5183 and subject to Commonwealth Act 138.



4. Interested bidders may obtain further information from the BAC Secretariat and inspect the Bidding Documents at the address given below during 8:00 a.m. - 5:00 p.m., Monday-Friday.



5. A complete set of Bidding Documents may be purchased by interested Bidders on May 8 - June 4, 2019 from the address given below and upon payment of the applicable fee for the Bidding Documents, pursuant to the latest Guidelines issued by the GPPB, in the amount indicated in the above table.

The WDH will hold a Pre-Bid Conference on Tuesday, May 21, 2019 at 9:00AM which shall be open to all interested parties.



6. Bids must be duly received by the BAC secretariat at the address indicated below. All Bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause 18. Bids will be opened in the presence of the Bidders- representatives who choose to attend at the address below. LATE BIDS SHALL NOT BE ACCEPTED.



7. Wao District Hospital reserves the right to accept or reject any bid, to annul the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.
