representative or they may also use this BPI Quotation/Canvass Form duly signed by their authorized representative

and with valid ID (government issued ID or company ID).

3. Any specifications other that those required/stated in this form shall not be considered in the evaluation of bid.

4. Quotation/s must include all kinds of taxes for the item(s)/services listed hereunder, including delivery charges.

5. Always indicate the brand name, model/unit, and detailed specification of the offered item(s) or product(s).

6. Price quotation(s) submitted shall be valid thirty (30) days from the deadline for submission of quotations.

7. Terms of Payment within thirty (30) days from delivery of item/s.

8. The supplier is required to affix his/her signature, across the flap of the sealed envelope if to be submitted thru

courier or mail to BAC Office, Bureau of Plant Industry BPI Administrative Building, 692 San Andres St., Malate,

Manila or thru electronic mail at bpibac@yahoo.com.
