c. Latest Income Tax Return filed thru EFPS;
d. Notarized Omnibus Sworn Statement (Must use the GPPB-prescribed template under
RA 12009 (NGPA). Submissions using the old RA 9184 template will be rejected. For
corporations/partnerships, attach a notarized Special Power of Attorney, Board or
Partnership Resolution, or Secretary's Certificate, whichever is applicable)
