SUPPLY AND DELIVERY OF EYE ESSENTIAL MEDICATIONS
UNDER IB NO. 26-77
1) The Department of Health Bicol Center for Health Development (DOH Bicol CHD), through the General Appropriations Act (GAA) intends to apply the sum of Nine Hundred Ninety-Nine Thousand Seven Hundred Forty-Five Pesos Only (Php 999, 745.00) being the Approved Budget for the Contract (ABC) to payments under the contract for Supply and Delivery of Eye Essential Medications under IB No. 26-77. Bids received in excess of the ABC shall be automatically rejected at bid opening.
2) The DOH Bicol CHD now invites bids for Supply and Delivery of Eye Essential Medications under IB No. 26-77. Delivery of the Goods is required on the expected delivery date stated in Section VI, Schedule of Requirements. Bidders should have completed, at least five (5) years from the date of submission and receipt of bids, contracts similar to the Project.
