2. Certificate of Good Manufacturing Practice (cGMP) from FDA

3. Batch release certificate from FDA

4. If the supplier is not the manufacturer, certification from the manufacturer that the supplier ia an authorized distributor/ dealet of the product/ items

5. License to Operate (LTO) from FDA with list of sources (whether it is a manufacturer, importer, seller, or distributor

Additional Requirements:

1. Expiration date must be 18months and above UPON DATE OF DELIVERY/ DELIVERIES
