REQUEST FOR QUOTATION (RFQ)
The National Museum of the Philippines, through the Bids and Awards Committee, invites all eligible suppliers to bid for:
SUPPLY AND DELIVERY OF CAPIZ SHELL LAMP HEAD AND HOLDER FOR THE CAPIZ SHELL LAMP HEAD
Approved Budget for the Contract (Total ABC): PhP 88, 900.00
LOT NO. ITEM DESCRIPTION Unit & Quantity
1 Capiz Shell Lamp Head (16-- diameter) for lamp post 3 sets
Holder for the Capiz Shell Lamp head (2mm wrought iron) 35 sets
The procurement will be conducted through Alternative Mode of Procurement - Negotiated Procurement (Small Value Procurement), Section 53.9 of the 2016 Revised IRR of R.A. 9184 otherwise known as the -œGovernment Procurement Reform Act-.
In view thereof, The National Museum Bids of the Philippines and Awards Committee requests the submission of your Price Quotation/Bid Proposal for the above project on or before October 22, 2019, not later than 10:00 A.M.
Kindly place your quotation/bid proposal in a SEALED ENVELOPE together with THREE (3) copies of the following:
1. Mayor-s /Business Permit,
2. PhilGEPS Registration Number,
3. Latest Income Tax (paid through the BIR Electronic Filing and Payment System (EFPS) with BIR Stamp)
4. DTI Certificate/SEC Registration Certificate
5. Omnibus Sworn Statement (duly Notarized with seal).
The aforementioned requirements shall be duly Certified True Copy with name and signature. The envelope shall bear the Title of the project to be bid, PhilGEPS Posting Reference Number, Name, Address and Contact Number of the bidder; and addressed to Dr. Luisito T. Evangelista, Chairman, Bids and Awards Committee, BAC Room, 3/F, National Art Gallery Building, National Museum, Padre Burgos St., Manila.
Quotations in excess of the ABC shall be automatically rejected.
