Please quote your lowest price for the following items enumerated below, taking into consideration the following:
PROCUREMENT TERMS & CONDITIONS
1. The Approved Budget for the Contract (ABC) is P173, 200.00
2. Price Quotation should be inclusive of the 5% final Vat and expanded withholding tax (EWT) of 1% VAT
3. Price Quotation should be valid with in (7) calendar days from the deadline of submission of RFQ
4. Evaluation of the quoted price will be conducted "per lot basis"
5. Delivery period is within (10) calendar days from receipt of Notice of Award/Purchase Order
6. If awarded the contract, processing and payment shall be made with in five (5) working days after complete delivery and acceptance of the items.
7. Refusal to accept an award maybe ground for imposition of administrative sanction under Rule XII of the IRR of RA 12009.
8. This request for Quotation must be submitted sealed to the above address not later April 06, 2026
