REQUEST FOR QUOTATION



The Logistics Support Service Bids and Awards Committee (LSS BAC) invites all interested eligible suppliers or manufacturers to submit proposals for the supply and delivery of the following goods pursuant to Rule XVI, Section 53 of the Revised-IRR of R.A 9184.

The goods must be delivered within the specified number of days indicated in the Schedule Requirements upon receipt of the Notice to Proceed. Suppliers, distributors, manufacturers shall submit their best and final price quote enclosed in a sealed envelope not later than September 5, 2019 at 9:00 AM at the LSS Building, Conference Room, 2nd Floor Camp BGen Rafael T Crame, Quezon City, together with the following documentary requirements. The price validity shall be for a period of 120 calendar days:



a. PhilGEPS Platinum Certificate of Registration;

b. Mayors Permit;

c. SEC/DTI/CDA Registration;

d. Tax Clearance;

e. Audited Financial Statements;

f. Annual Income Tax Returns;

g. Latest income and business tax returns

h. Omnibus Sworn Statement;

i. Special Power of Attorney;

j. Joint Venture Agreement (if applicable);

k. Pre-test result of the product being offered using the PNP Test Parameters under Memorandum Circular No. 2014-043 from any NIJ accredited testing facility here or abroad (for the Waistcoat Vest Level III-A with Upgraded Plates Level IV only)

l. Statement of Single Largest Completed Contract similar to this Project, equivalent to at least fifty percent (50%) of the ABC and its proof such as Purchase Order, Contract, Acceptance Report and Official Receipt;

m. Statement of all ongoing contract/s, Government and Private;

n. Certificate of Net Financial Contracting Capacity (NFCC);

o. Schedule of requirements;

p. Conformity to the approved NAPOLCOM Technical Specifications; and

q. Brochure or Prototype.

The LSS-BAC reserves the right to accept or reject any quotation/s, to annul the negotiation process, and to reject all quotation/s at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.
